Nevuses generally vary in size, shape, hairiness and regularity of the surface and colours range from light brown to black. Some have no hairs on them, while others may have very thick ones. Sometimes nevus on the face can be aesthetically disturbing and also some nevus can remain for years without any discomfort on some parts of the body.

The reason why nevuses are different from the skin is because excessive accumulation of melanocytes that produce the melanin hormone in the epidermal layer of the skin or the production of excess melanin and nevus are formed as a result of exposure to the sun. The risk of cancer in nevus is higher in congenital ones. The most common of these cancers is a type of skin cancer called malignant melanoma.

The incidence of this type of cancer is increasing rapidly all over the world. This cancer can grow under the skin. Progression is important for the grading of cancer. The further it goes under the skin and the more blood vessels and lymph system are infected, the greater the risk of life.

Therefore, it is very important to consult a doctor early in such cases. If there is an increase in colour change, bleeding, enlargement and irregularity on the edges of the mole, a doctor should be consulted immediately. Because those changes are signs of the onset of cancer and if not intervened early, period of recovery is rather difficult. 

There is a false belief amongs the people which is if those nevusus are cut and removed then they would spread.This belief does not go beyond superstition and can negatively affect the patient's life and if they act according to that belief, consulting a plastic surgeon would be delayed and in case of a possible skin cancer, it would be too late. Even if the necessary intervention is made, the lesion cannot be completely removed.

Therefore, the lesion will grow inside again, and if there is cancer, it spreads to different organs of the body by using the blood or lymph system, which is called metastasis. In order to avoid those necessary medical treatment should be received as soon as possible.The risk of developing malignant melanoma is 1.5% in overall population. In other words, one out of every 70 people has a lifetime risk of developing this type of cancer.

Unfortunately nowadays, damage in the ozone layer and vulnerability which is created by exposure of UV rays increase the risk. When people are exposed to harmful rays, mutations occur which are the cellular changes in DNA structurel codes. These mutations cause differences in skin cells and can develop into cancer cells that are very different from the surrounding tissues.In order to remove those nevusus, it is necessary to apply to a Plastic Surgeon specialist. 

Nevus removal can be done in medically appropriate and adequate settings and is performed under local anesthesia with a fine-tipped injector, local anesthesia is given around and under the mole. Then lesions are removed from the skin with a scalpel or cautery and sutured in an aesthetic way so that there is no visible scars. If the lesion is very large then it is closed with flap surgery or skin patch (skin graft).

The removed nevus is sent to the pathology laboratory to determine whether it is benign or malignant or does the lesion spreading to blood vessels?

Have cells belonging to this lesion entered the subcutaneous lymph system? If the lesion has descended under the skin and how far at what level? such questions needs answering. If the result is malignant, additional treatments such as chemotherapy and radiotherapy are needed that aimed to destroy cancer cells.  

Some nevuses are not harmful, if it is on the face then it would cause just an aesthetic problem and can be removed with laser. There is a single session method that does not need stitches and during the procedure Erbium Oil and Carbon Dioxide Lasers are used. After the procedure, patients can return to their regular social life. Fat deposits around eyes can also be removed easily with this method that is done under local anesthesia and painless.

There is pinkness within 4-6 days of removal and it disappears within 1-2 months. This method is not used in deep skin lesions. In such cases, the lesion is surgically removed.As you can see, nevus removal processes have no harm to the body, but on the contrary, they have many benefits. Therefore, when a change in the colour and edge pattern is detected on the surface a plastic surgery specialist should be consulted without any delay and it should be removed from the skin if necessary.
