The quality of the skin is the main reason in these deformations. The problem is the reduction of subcutaneous fat layer and skin tightness. The skin could not adapt to this and it becomes loose. Collagen and elastic fibers in the skin, which provide the flexibility of the skin, expand and lengthen with weight gain. If the weight loss process is done very slowly, these fibers may gradually shorten and return to their old form. Pregnancy could be an example to this.

This excessive skin on the body could lead to difficulty in choosing clothes, poor quality of life and eventually social isolation.

Post bariatric surgery is performed to eliminate these deformities that occur after losing too much weight. With this method, it is aimed to remove the bad images after weight loss by putting the body back into the desired form.

Who is Eligible for Post Bariatric Surgery?

In order for post bariatric surgery to be performed, the patient must reach the targeted weight with the roadmap drawn for him through diet, sports and obesity surgery. In addition, the patient's weight should be stabilized after bariatric surgery. This process takes approximately 6 to 18 months. If weight loss continues rapidly after the surgery, sagging and looseness may occur again.

What distinguishes post bariatric surgery from other body shaping surgeries is that there are deformations in almost every part of the body due to rapid weight loss. These patients are operated more than once and in several sessions. 

Before Post-Bariatric Surgery

Any healthy individual who has had bariatric surgery can also have these surgeries.However, if there are diseases that can affect surgeries, body shaping surgery can be performed after these are taken under control.

Depending on the type of surgery performed after weight loss operations, patients may have vitamin deficiencies and low blood hemoglobin levels.

All these are important factors for wound healing, vitamin, protein and blood values should be at normal levels in order for these wounds to heal quickly and well.

In addition, smoking is an important factor that negatively affects wound healing.Quitting smoking at least three weeks in advance allows for better wound healing and more acceptable future scars. 

  • Before the operation, the person who has completed weight loss should have reached his basal weight. Detailed examinations should be done by the physician before the operation.
  • It should be known that this surgery would be performed in a few sessions. 
  • Apart from this, routine blood tests and anesthesia examinations are performed as in all surgical operations.
  • The person's drug use, disease, etc. should be shared with his/her doctor. 

How is the Surgery Planned and Performed?

It is not possible to complete the post-bariatric surgical body shaping in a single session. For these type of surgeries, a taylor made combination is created and the operation is planned.

When planning surgery for sagging skin, a surgical plan is created by classifying the regions within themselves. 

Waist and Abdominal Area

When planning these surgeries, it is decided from which area to start after examination. However, in general, this surgery is preferred the first on the lineby many patients. Abdomen, waist and hips are completely straightened. However, these surgeries are usually not performed in a single session.

  • Complete tummy tuck
  • Correction of laxity in the muscles, if necessary
  • Stretching and prosthesis for sagging and flattening in the hip area could be applied.

Arm and Breast Area 

Due to sudden weight loss, breast tissue loses significant fat and tends to sag downwards quickly. In the post-bariatric period, this is accompanied by the arm and back region.According to the patient's needs, arm lift, breast lift and sagging behind the bra can be intervened in the same session.

However, depending on the condition of the breasts, a silicone prosthesis can be added to the breast lift surgery or the surgery can be performed in a few sessions. Depending on the patient's condition, the doctor will decide during the examination.  

Leg Area 

The sagging skin excesses on the upper parts of the legs are collected on the inner sides of the legs. Here, the scars are hidden longitudinally in the inner region of the legs facing each other.This surgery can also be performed in combination with tummy tuck surgery. 

Face Lift 

After weight loss operations, serious injuries may occur in the eyebrow, chin and neck region, in this case the operation to be performed is the same as the classical facelift surgery, the operation begins with the incisions to be made in front of and behind the ear, the facial skin and neck skin are lifted, the subcutaneous tissues and skin are tucked behind the ear. Excess tissues are removed and sutured. In addition, the eyebrow area is hung upwards. The scars are hidden in front and behind the ear. A face corset is worn for about three weeks after the surgery, so that blood accumulation under the skin is prevented, and these corsets also allow the new facial skin to be shaped.

This surgery is sometimes supported with fillings depending on the condition of the patients. 

Hip Lift

One of the areas that negatively affect women after weight loss surgeries is the hip region. The drooping areas here create difficulties in choosing a dress. In the surgery to be performed, it starts with an incision to be made from the waist region. This incision can be hidden under underwear. As the hip skin is opened upwards, the hip tissues are supported and shaped with the excess tissues in the waist area. Patients are advised not to lie on their back for a few weeks until the wound heals, these surgeries are usually not performed in the same session as tummy tuck surgery. 

Post-Bariatric Surgery Period

  • Operations are performed under general anesthesia and take between 2-4 hours depending on the procedure to be performed.
  • If the muscles under the skin are intervened during the operation, the patient may stay in the hospital for two or three days so that he can walk more comfortably.
  • If the muscles under the skin are not touched, a one day hospital stay is sufficient.
  • Patients can take short walks in the room after a certain period of time on the first day of the surgery.
  • It is recommended to use a corset for 6 weeks in order to make the post-operative remodeling specific to the treated area more prominent and to regress more rapidly in surgical edema.
